An affidavit having been made and filed showing that the Natural Parent in the above-entitled cause is a non-resident individual, other than non-resident individual fiduciary who has appointed a statutory agent; or cannot be found, and that the diligence has been used without effect to ascertain the location of the party to be served; or has had no contact with the child since the child's birth, and all due dilligence has been used in seeking his information. Mailings were sent to those addresses associated with him in public records, and phone calls were made to numbers associated; and the last known mailing address of Natural Father/Mother is as follows:
